What is a GP Exercise Referral Scheme?
A GP Exercise Referral Scheme is a program where GPs or other healthcare professionals refer individuals to tailored exercise programs. These programs are designed to help improve health, manage specific conditions, and enhance quality of life through regular physical activity. The referral is often aimed at those who have medical conditions or lifestyle factors that make it difficult to engage in exercise on their own.

Who Can Be Referred?
GP Exercise Referral schemes are suitable for individuals who:
>Are living with long-term conditions such as diabetes, hypertension, arthritis, or obesity.
>Have mental health conditions like depression or anxiety that may benefit from exercise.
>Are at risk of chronic diseases due to lifestyle factors e.g sedentary lifestyle.
>Are recovering from surgery or illness and need structured rehabilitation.
>Are looking for expert guidance to safely increase their physical activity.

How Does the Referral Work?
1.Consultation with your GP
>If you’re experiencing health concerns that could benefit from exercise, talk to your GP or healthcare professional.
>They will assess your health, discuss your needs, and determine if an exercise referral program is appropriate for you.

2. Referral to an Approved Exercise Provider
>Once referred, you’ll be directed to a certified exercise professional, typically at a local leisure centre, gym, or fitness facility of your choosing.
>You will attend a one-to-one consultation myself (GP referral instructor).
>As a GP Referral professional, I will design a personalized exercise plan based on your medical history, abilities, and fitness goals.
>Referrals are 12 weeks long and include a personalized exercise plan, gym membership/entry at The Movement Gym, check ins, support and supervised group gym sessions with me (your GP referral instructor).

3.Personalized Exercise Plan
>Your exercise program will include safe and effective activities tailored to your needs.
>You’ll be supported and monitored throughout your program to ensure progress and adjust as necessary.
> You will attend supervised group sessions

4.Ongoing Support and Monitoring
>Regular follow-ups are scheduled to track your progress and provide motivation.
>The program can be extended if necessary, with continued support to help you maintain a healthy lifestyle long-term.

5. Completion Support and Progression
Upon completion of your 12-week GP exercise referral, you can:
>Continue with your programme.
>Be eligible to receive discounted gym membership.
>Purchase personal training sessions with your GP exercise referral instructor (me).
>Purchase a new training programme.
